Enrollment Information Sheet 

By August 15th

  1. Fill out and return Registration Form (26-27), including fee.

  2. Fill out and return Child Information Record

  3. Fill out and return Handbook Receipt & Volunteer Statement Contract

  4. Fill out and return Health Appraisal

  5. Send 1st months tuition ($190/mo)

By school starting

  1. Perform Ages & Stages Questionnaires: Third Edition and Social Emotional (choose to take both, or complete at separate times but both need to be completed)

4’s Preschool (4 Half-Days)
